How to Order a Death Certificate

There are three ways to get a certificate:

• Online - one business day.
• Walk-in - approximately 15 minutes
• By mail - 2-3 business days.

Cost – Death Certificates cost $21 for each copy, and $4 for each additional copy ordered at the same time, with an additional processing fee for internet orders.

Information Required

• The name on the record being requested.
• Date of death
• Father's full name.
• Mother's full maiden name.
• Name of the county and city where the birth occurred.
• Requestor's name, address, and daytime telephone number.
• Requestor's relationship with the person whose record is being requested.
• An explanation of the purpose of the request.
• Current government-issued photo identification of the requestor
