The City of Austin (City) seeks applications in response to this Request for Applications (RFA) from qualified providers (Applicants) with demonstrated experience in providing community-based health prevention and education programs for African-American, Latino, Asian American, Immigrant, Elderly, and/or Lesbian, Gay, Bisexual, Transgender (LGBT) populations. The City requests applications that address disparate health outcomes for each of the populations identified, through community-based interventions.
